We are accepting new patients

Call (513) 860-4600 to reach the front desk. New patient appointments are booked by phone so we can match you with a provider and answer your questions while you are on the call. Our hours are Monday through Friday, 7:30 AM to 4:30 PM.

We welcome patients of all ages, whether you need a family doctor for everyday care or you are looking for help with memory changes or other complex symptoms. Most major insurance accepted, call to verify.

What to bring to your first visit

  • Your completed New Patient Registration Form
  • A photo ID
  • Your insurance card
  • A list of the medicines you take, including vitamins and supplements

Download the registration form below, fill it out at home and bring it with you. Completing it ahead of time helps your visit start on time.

Your first visit

  1. Check in. Arrive 15 minutes early. The front desk will check you in and ask for your registration form, photo ID and insurance card.
  2. Paperwork review. A member of our care team goes over your forms, your medicines and your health history with you.
  3. Meet your provider. Your provider talks with you about your health and your concerns, reviews your history and does a physical exam as needed.
  4. Plan and next steps. You leave with a plan. That may include lab work as needed, a referral to a specialist or a follow-up visit.
  5. Follow-up and portal access. Ask the front desk about the patient portal, where you can send messages to your care team and see test results. For anything else, call us.

If your first visit is about memory concerns

If you are coming in about memory loss or confusion, it helps to bring a family member or close friend. They can share what they have noticed and help you remember the plan. Learn more about our memory and cognitive health care.

Choosing a provider

Our three providers all see patients of all ages for sick visits, preventive care and long-term conditions. Dr. Chris Diatte is a board-certified family physician. He is joined by two board-certified family nurse practitioners. Read about each one on our providers page. If you're not sure who to see, the front desk can help you choose when you call.

Insurance

Before your first visit, call us so we can check your plan with you. You can also call the number on the back of your insurance card and ask if Dr. Diatte is a provider listed on your insurance plan. Learn more on our insurance and billing page.
