The short version

Your medical information is private, and a federal law called HIPAA protects it. HIPAA also gives you rights over that information, such as the right to see your records and ask us to fix mistakes. Every medical practice must give patients a written notice that explains how it uses and shares medical information and what your rights are. That notice is called a Notice of Privacy Practices.

A copy of our full Notice of Privacy Practices is available at the front desk, and we can mail you one. Call (513) 860-4600.

What a Notice of Privacy Practices is

Under the HIPAA Privacy Rule, a health care provider's notice must explain:

  • how the practice may use and share your medical information
  • your rights over your medical information and how to use them
  • the practice's legal duties, including that it is required by law to keep your medical information private, to give you this notice, and to tell you if a breach affects your information
  • how to file a complaint if you believe your privacy rights were not respected

The notice must be available to anyone who asks for it.

Your rights under HIPAA

This is a plain-language summary of the rights the HIPAA Privacy Rule gives you. Our full notice explains how to use each one at our office.

  • See and get a copy of your records. You can ask to look at and get a copy of your medical and billing records.
  • Ask for corrections. If you think something in your record is wrong or missing, you can ask us to change it.
  • Get a list of certain disclosures. You can ask for a list of certain times we shared your medical information.
  • Ask us to limit what we use or share. You can ask us not to use or share certain information. We are not always required to agree, and the full notice explains when we must.
  • Ask for confidential communications. You can ask us to contact you in a certain way or at a certain place, such as a different phone number or mailing address.
  • Get a paper copy of the notice. You can ask for a paper copy of our Notice of Privacy Practices at any time, even if you already received it another way.
  • File a complaint. If you believe your privacy rights were violated, you can complain to us or to the U.S. Department of Health and Human Services. We will not retaliate against you for filing a complaint.

How to file a complaint
