Changes worth talking about

Everyone forgets things now and then. But some changes are worth a conversation with a doctor, especially if they are new or getting worse. These include:

  • Asking the same question or telling the same story again and again
  • Misplacing things more often than before
  • Trouble with familiar tasks, like following a recipe or paying bills
  • Trouble finding the right word in conversation
  • Getting lost in a place you know well
  • Changes in mood, judgment or interest in usual activities

These changes have many possible causes, and some of them can be treated. An evaluation is the first step toward understanding what is going on. Sometimes a family member notices the changes first. That's a good reason to call.

What the evaluation includes

Your evaluation starts with a detailed history. We ask about when the changes started, how they affect daily life, and your overall health. A family member or friend who knows you well can add a lot here.

Your provider will also do a physical exam and a cognitive screening, and review your medications and supplements, since some can affect memory and thinking. We'll ask about sleep, mood, activity and nutrition. Lab work is ordered as needed, based on your history and symptoms.

How our protocol-based approach works

Dr. Diatte is trained in the Bredesen Protocol®, sometimes called ReCODE. It is a structured approach that looks at many possible contributors to memory problems at once, rather than one at a time.

For you, that means a personalized plan that looks for and addresses factors that can contribute to cognitive decline. Depending on your evaluation, your plan may look at:

  • Sleep quality and sleep problems
  • Physical activity
  • Nutrition
  • Medications that may affect thinking
  • Other health conditions, such as blood sugar, blood pressure or thyroid problems
  • Stress and social connection

Plans take effort and follow-up. We check in at regular visits, see what's working, and adjust the plan over time.

What the research shows

We want you to have honest information. The evidence for the Bredesen Protocol comes mostly from small, uncontrolled reports. Larger studies are ongoing. Some researchers have publicly questioned the approach and called for stronger trials (KFF Health News reported on these concerns).

Many parts of a plan, such as better sleep, regular activity and good control of other health conditions, are part of good care for anyone. We'll talk with you about what is known, what isn't, and what to expect. Results vary from person to person.

Family members are welcome

Memory care works best as a team. A spouse, adult child or close friend is welcome at every visit. They can share what they've noticed, help remember the plan, and support changes at home. With your permission, we'll keep them informed.

Patients come to us from across Greater Cincinnati, Dayton, and Butler and Warren counties. If you're traveling, tell us when you call and we'll help plan your visits.

When to see a specialist

Sudden changes are an emergency. If you or someone you're with has sudden confusion, trouble speaking, weakness or numbness on one side, a severe headache, or a sudden change in alertness, call 911 right away.

We may refer you to a neurologist if memory or thinking is getting worse quickly, if symptoms started at a younger age, if there are changes in walking, balance or movement, or if your evaluation points to a condition that needs specialist testing or treatment. We work alongside neurologists and other specialists, and we keep caring for you as your family doctor.

Billed to your health insurance

Coverage varies by plan.

Care for these conditions is often cash pay through a functional medicine practitioner, so patients pay for every visit and lab test out of pocket. At Beckett Ridge Family Medicine, we offer these services and bill your traditional health insurance, so you can use the benefits you already have.

How billing works

We bill your health insurance for office visits and lab work, including functional medicine care. Coverage depends on your plan, and some tests, treatments or supplements may not be covered. Call us before your visit and we'll help you check your benefits. You are responsible for any charges your plan doesn't cover.

We work alongside neurologists, infectious disease specialists and other specialists, and we'll tell you when a specialist visit is the right next step.

No. The Bredesen Protocol is not a cure for Alzheimer's disease or any other cause of memory loss. It is a structured approach to building a personalized plan that looks for and addresses factors that can contribute to cognitive decline. Yes. We encourage it. A family member or friend can share what they've noticed and help with the plan at home.

Some patients do. We'll refer you to a neurologist if your symptoms are changing quickly, started at a younger age, or need specialist testing. If you already see a neurologist, keep seeing them. We work alongside your other doctors.

No. Don't stop or change any medication on your own. Bring a list of everything you take, and we'll review it with you and your other doctors.

It depends on your plan and how things are going. Most plans include regular follow-up visits so we can review results, see what's working and make changes. We'll set a schedule with you, and we keep in mind how far you travel.
