Sports physicals from a team that knows athletes

A sports physical, also called a pre-participation physical evaluation, checks whether it's safe for a student to play and looks for anything that should be addressed before the season starts. At Beckett Ridge Family Medicine, these exams are part of our everyday family care.

Dr. Chris Diatte is board certified in Family Practice and in Sports Medicine, and he completed a Sports Medicine Fellowship with St. Elizabeth's Hospital in Dayton, Ohio. Over the years he has volunteered with high school athletic teams, so he understands what young athletes and their parents deal with during the season.

We care for Lakota-area families from West Chester and Liberty Township, as well as students from Mason, Fairfield and nearby schools.

The OHSAA pre-participation form

Ohio high school athletes need a completed pre-participation physical exam (PPE) form before they can practice or compete. The Ohio High School Athletic Association posts the current form on its Pre-Participation Physical Exam Form page.

Before your visit:

  • Download and print the form for the current school year.
  • Fill out the history pages with your student. Answer every question as completely as you can, especially the heart health and family history questions.
  • Sign the sections meant for the student and parent or guardian.

Your school's athletic office may use its own system for collecting forms, so check with them about deadlines and how to turn the form in. Our provider completes the exam portion during the visit.

What to bring

  • The completed and signed OHSAA form, or the form your school, camp or employer requires
  • Glasses or contact lenses, if your student wears them, for the vision check
  • A list of current medications, including inhalers and allergy medicines
  • Family history, especially any relatives who had heart problems or died suddenly at a young age
  • Your insurance card

What the exam covers

Your provider reviews the health history, checks vital signs and vision, and does a physical exam with attention to the heart, lungs, joints and overall fitness to play. We also ask about past injuries, concussions, and any symptoms during exercise, such as chest pain, fainting or unusual shortness of breath.

If something needs a closer look, we'll explain why and talk through next steps. Sometimes a follow-up visit or a specialist referral is needed before a student is cleared to play.

School, camp and work physicals

We also complete physicals for school entry, summer camps, scouting and jobs. Each organization has its own form, so bring the one you've been given. If you're not sure what's needed, ask the school, camp or employer for their form before you come in.

Sports physicals and well-child visits

A sports physical focuses on safe participation. A well-child visit is broader and covers growth, development and overall health. Ask our front desk whether it makes sense to schedule a well-child visit for your student as well. If an athlete gets hurt during the season, we also see joint pain and sports injuries.

Step by step

What to expect at your visit

  1. Call and print the form

Call (513) 860-4600 to schedule. Print the current OHSAA form, or the form your school, camp or employer requires, and complete the history pages at home.

  2. Check in with a parent or guardian

Plan for a parent or guardian to come with students under 18 to help with history questions. Bring the signed form, glasses or contacts, a medication list and your insurance card.

  3. Vitals and vision

We check height, weight, blood pressure, heart rate and vision.

  4. History review and exam

Your provider goes over the health history and examines the heart, lungs, joints and overall readiness to play.

  5. Form completed

If your student is cleared, your provider completes the exam portion of the form. If more evaluation is needed first, we explain the next steps.
